Our Services in Suffolk County

Getting Started in Suffolk County

If your child has Medicaid

A Health Home care manager checks whether your child qualifies for HCBS and builds the plan of care with you. Call us and we can start both.

If your child does not have Medicaid yet

Suffolk County takes Medicaid applications by phone at 1-855-355-5777 (NY State of Health) or in person at a Department of Social Services center: 605 Old Country Road in Riverhead, 80 Middle Country Road in Coram, 200 Wireless Boulevard in Hauppauge or 2 South 2nd Street in Deer Park.

A child who may qualify for the Children's Waiver but has no Medicaid is referred to C-YES, now run by the New York State Department of Health: 1-833-861-4467. For some children, Medicaid counts only the child's own income (Family of One budgeting).

Early Intervention for babies and toddlers

In Suffolk County, all Early Intervention referrals go to the Department of Health Services, Division of Services for Children with Special Needs, at 3500 Sunrise Highway in Great River. Its referral line is (631) 853-3100. From age 3, each of Suffolk's school districts has a Committee on Preschool Special Education (CPSE) that decides on preschool services.

Developmental disability services (OPWDD)

To apply for OPWDD services, call the OPWDD Infoline at 866-946-9733 and ask for the Front Door office for Suffolk County.
