Getting Started in Queens

If your child has Medicaid

A Health Home care manager checks whether your child qualifies for HCBS and builds the plan of care with you. Call us and we can start both.

If your child does not have Medicaid yet

Most New York City families apply for children's Medicaid through NY State of Health at 855-355-5777. A child under 18 who has a waiver, or has been told they need long-term care services, can apply online through ACCESS HRA. The HRA Medicaid Helpline is 1-888-692-6116, and HRA's Queens Medicaid offices are at 32-20 Northern Boulevard in Long Island City and 165-08 88th Avenue in Jamaica.

A child who may qualify for the Children's Waiver but has no Medicaid is referred to C-YES, now run by the New York State Department of Health: 1-833-861-4467. For some children, Medicaid counts only the child's own income (Family of One budgeting).

Early Intervention for babies and toddlers

For a child under 3, call 311 and ask for Early Intervention, or use the city's online referral portal. You do not need a doctor's referral. Questions about a plan or services already in place go to the Queens regional office at 90-27 Parsons Boulevard in Jamaica, 718-553-3954. From age 3, preschool special education is handled by the Department of Education's Committee on Preschool Special Education (CPSE).

Developmental disability services (OPWDD)

To apply for OPWDD services, call the OPWDD Infoline at 866-946-9733 and ask for the Front Door office for Queens.
