Home / Areas We Serve / Manhattan
Yeled Long Island works with Manhattan families raising children with serious emotional, behavioral or medical needs, from Lower Manhattan to Inwood.
A Health Home care manager checks whether your child qualifies for HCBS and builds the plan of care with you. Call us and we can start both.
Most New York City families apply for children's Medicaid through NY State of Health at 855-355-5777. A child under 18 who has a waiver, or has been told they need long-term care services, can apply online through ACCESS HRA. The HRA Medicaid Helpline is 1-888-692-6116, and HRA's Manhattan Medicaid offices are at 115 Chrystie Street and 4055 10th Avenue.
A child who may qualify for the Children's Waiver but has no Medicaid is referred to C-YES, now run by the New York State Department of Health: 1-833-861-4467. For some children, Medicaid counts only the child's own income (Family of One budgeting).
For a child under 3, call 311 and ask for Early Intervention, or use the city's online referral portal. You do not need a doctor's referral. Questions about a plan or services already in place go to the Manhattan regional office at 42 Broadway, Suite 1611, 212-436-0900. From age 3, preschool special education is handled by the Department of Education's Committee on Preschool Special Education (CPSE).
To apply for OPWDD services, call the OPWDD Infoline at 866-946-9733 and ask for the Front Door office for Manhattan.
Yes. We work with families across Manhattan, and in the other boroughs of New York City and in Nassau and Suffolk counties. Our staff come to your home or meet your child in the community.
The city's Manhattan Early Intervention regional office is at 42 Broadway, Suite 1611, and its phone is 212-436-0900. To refer a child, call 311 and ask for Early Intervention.
Health Home care management and HCBS are Medicaid services, so yes. If your child is not enrolled yet, follow the steps above or call us and we will walk you through them. Our step-by-step guide to the Children's Waiver explains who qualifies.
