Whether you need expert advocacy, tailored resources, or emotional support, our team is here to guide you every step of the way.
Support for Children with Emotional, Behavioral, or Complex Medical Needs
If your child, aged 0-21, is struggling with emotional or behavioral issues, or is facing complex medical needs, there are Medicaid-based programs that can help provide the support your family needs.
These programs are designed to assist both the child and their caregivers, offering access to essential resources and services to improve your child's well-being and support your caregiving efforts.
Services Your Child May Be Eligible For:
Dedicated Care ManagerA care manager will work with your family to coordinate services and ensure your child receives the right support. This personalized care helps manage emotional, behavioral, or medical challenges and ensures your child has access to all available resources.
After-School SupportIf your child needs additional support outside of school hours, our programs provide after-school care designed to foster development and address specific challenges. This may include activities that support social skills, emotional regulation, and learning.
Caregiver SupportBeing a caregiver for a child with complex needs can be overwhelming. Our programs offer support for caregivers, including resources, training, and respite care to help you manage your responsibilities and take care of yourself.
Initial Visit
Once you are assigned a Care Manager, they start by listening to your concerns and understanding your child's unique needs to determine how we can best support them.
Evaluation
The Care Manager conducts an in-depth assessment of your child to gather insights and tailor our approach.
Support
Eligible children will be connected to additional supports based on their needs and the Plan of Care. Those supports may include respite, CFTSS, CFASS, pre-vocational, etc.
Ongoing Support
We continuously monitor progress and adapt the plan to ensure consistent growth, celebrating every milestone along the way.
Children's Health Home
Our Health Home program links families with a broad network of healthcare professionals, ensuring effective management of health needs through a committed care manager who oversees service coordination and resource access.
Home & Community Based Services (HCBS)
Home and Community-Based Services (HCBS) offer Medicaid-funded support to children with medical fragility or those at risk, delivering care in home and community settings to prevent the need for more restrictive institutional environments.
Caregiver Family Advocacy Supports Services (CFASS)
Caregiver and Family Advocacy and Support Services provide families with vital resources to help children with disabilities succeed in their homes and communities. Through education, guidance, and access to essential tools, these services empower caregivers to meet individual needs and enhance family support systems.
Respite Services
Respite care provides temporary relief for caregivers of children with disabilities, ensuring the child’s safety, supervision, and engagement to support their health and development. Delivered by trained professionals, this care can be offered at home, in community settings, or other approved locations.
Pre-vocational
Services
Preparing youth for successful employment involves equipping them with essential job readiness skills and familiarizing them with workplace expectations. Through targeted training, young individuals gain the necessary competencies to thrive in a professional environment, including communication, time management, teamwork, and problem-solving.
Community Habilitation Services
Supporting youth in the development and enhancement of daily living skills is essential for fostering independence and well-being. This includes activities that promote self-care, personal safety, effective communication, problem-solving, and positive social interactions. By engaging in these skills-building exercises, youth learn how to manage daily tasks, navigate social environments, and make informed decisions, thereby increasing their ability to lead fulfilling and independent lives.

ABA Insurance Services
Our ABA Insurance program offers insurance-approved services to develop daily living skills, using a holistic, individualized approach that collaborates with families, providers, and schools.
Early Intervention Services
Our Early Intervention program supports children (0-3) in reaching developmental milestones, providing timely help to address delays and promote healthy growth.
Therapy Referrals
We offer school-based counseling, collaborating with families and teachers to support students' emotional development, help them overcome challenges, and reach their goals.
SEIT Referrals
We create inclusive learning environments tailored to each child’s needs. Our SEIT program helps preschoolers (ages 3-5) develop school-readiness skills, fostering future academic success.
