The Care Manager helps connect the child and caregiver to a variety of resources. Our program also ensures that your child receives the right care at the right time. By creating a comprehensive care plan and collaborating with a network of providers, we help reduce the stress of managing appointments and services, ensuring your child’s overall care is well-organized and effectively coordinated.

The Children's Health Home program includes a variety of services, such as:
Care management and coordination with doctors, hospitals, and specialistsAssistance in connecting to mental health or medical resourcesMaximizing access to resources and community servicesOngoing support and communication with families to ensure a seamless healthcare experience

The program is covered by Medicaid (and there is a Waiver process for children who do not have Medicaid).

Your assigned care manager will connect you to resources. They also conduct assessments and make face to face visits. They also help collaborate with your child's entire care team from school professionals to therapists to medical providers.
