Home and Community Based Services 

Yeled LI offers Home and Community-Based Services (HCBS) for children who are medically fragile or at risk due to mental health challenges. Through this program, children and their families receive support in a way that promotes independence and community involvement. Our HCBS services include CFASS, Respite care, Pre-vocational Services, and Community Habilitation, all designed to meet the specific needs of children while promoting development and well-being in a familiar environment.

HCBS 

The goal of HCBS is to allow children and youth to receive care in the comfort of their homes and communities, avoiding more restrictive settings like long-term care facilities or psychiatric inpatient placements. Through Medicaid funding, these services are provided in a way that aligns with each child’s cultural and developmental needs.
