Getting Started in Nassau County

If your child has Medicaid

A Health Home care manager checks whether your child qualifies for HCBS and builds the plan of care with you. Call us and we can start both.

If your child does not have Medicaid yet

Most families apply for children's Medicaid through NY State of Health at 855-355-5777. The Nassau County Department of Social Services, at 60 Charles Lindbergh Boulevard in Uniondale, handles Medicaid for long-term care; its Medicaid line is 516-227-8000.

A child who may qualify for the Children's Waiver but has no Medicaid is referred to C-YES, now run by the New York State Department of Health: 1-833-861-4467. For some children, Medicaid counts only the child's own income (Family of One budgeting).

Early Intervention for babies and toddlers

Early Intervention in Nassau County is run by the county Department of Health's Office for Children with Special Needs, at 60 Charles Lindbergh Boulevard, Suite 100, in Uniondale. Call 516-227-8661 to refer a child from birth to 2 years and 9 months. After that, preschool special education goes through your school district's Committee on Preschool Special Education (CPSE); the county's preschool program can be reached at 516-227-8692.

Developmental disability services (OPWDD)

To apply for OPWDD services, call the OPWDD Infoline at 866-946-9733 and ask for the Front Door office for Nassau County.
